  • Experience certificate in relevant field of work (civil, electrical, cremation services, etc.)

  • Educational qualification / technical certificate (for specialized work licences)

  • Bank passbook / cancelled cheque for financial details

  • Passport size photographs of applicant(s)

  • Labour licence (if applicable)

  • ESI & PF registration (if employees are engaged)

  • Undertaking / affidavit as per BBMP format

  • Any approvals from Pollution Control Board (for environmental/cremation-related works)
