- Medical Fitness Certificate (Form 1A) – Mandatory
- Proof of Residence & Identity – Same as above (Aadhaar, Voter ID, etc.)
- Passport-sized Photos – 4 to 6 copies
- Driver Training Certificate (Form 5) – From Government-recognized or RTO-authorized Heavy Vehicle Training School
- Police Clearance Certificate (in some states) – Especially for commercial vehicle license
